Research suggests that Vitex agnus-castus (chasteberry) has meaningful evidence supporting its use for cyclic mastalgia, the recurring breast pain many women experience before their periods, with two meta-analyses pooling data from dozens of clinical trials — including multiple randomized controlled trials — consistently finding that it reduces breast pain intensity and lowers elevated prolactin levels compared to placebo or active controls. A large real-world observational study of 1,700 women similarly reported that breast tenderness dropped dramatically over roughly three months of use, and a separate randomized controlled trial found it outperformed placebo for breast pain relief alongside flaxseed. However, the evidence carries notable limitations: many underlying studies have unclear or high risk of bias, most trials are short in duration, and the overall body of research calls for more rigorous and standardized investigation before strong conclusions can be drawn. Additionally, pharmacovigilance reviews and a 2006 clinical overview flag safety concerns, including breast- and reproductive-related adverse reactions and the herb's complicated, dose-dependent relationship with prolactin, which has led some researchers to caution against its use in breastfeeding women and to recommend closer monitoring of its side effect profile more broadly.
