Lactobacillus Acidophilus for Vaginal Health

Moderate evidence 11 studies

Research suggests that Lactobacillus acidophilus and related Lactobacillus strains play a meaningful role in maintaining vaginal microbiome balance, with several studies indicating that both oral and vaginal probiotic formulations containing these bacteria can support healthier bacterial profiles, as measured by improved Nugent scores and reduced dysbiosis-related symptoms. The evidence base includes small randomized controlled trials, observational studies, laboratory research, and reviews spanning several decades, with the general direction leaning supportive but with important nuances — for instance, one 2025 RCT found that a combination tablet containing ultra-low-dose estrogen plus L. acidophilus performed no better than a non-hormonal moisturizer in postmenopausal pessary users, and a 1993 review concluded that topically applied dairy-derived Lactobacillus products such as yogurt lack convincing clinical support. A systematic review and product survey both highlight a significant mismatch between the strains shown to be clinically effective for vaginal health and those actually present in widely available commercial products, suggesting that not all L. acidophilus-containing supplements are equivalent. Overall, studies indicate genuine potential for Lactobacillus acidophilus as part of a vaginal health strategy, but the body of evidence remains limited by small sample sizes, varying formulations and delivery methods, and a lack of large-scale trials, meaning firm conclusions are difficult to draw at this time.
