Research suggests that fenugreek seed has a long history of traditional use as a galactagogue and remains one of the most commonly self-reported herbal remedies among breastfeeding mothers, but the clinical evidence supporting its effectiveness is inconsistent and generally of low quality. Studies include two meta-analyses, multiple systematic reviews, and a small number of randomized controlled trials, with findings ranging from modest increases in milk volume to no statistically significant effect compared to placebo; a 2018 network meta-analysis found a positive signal versus placebo, while a 2020 Cochrane review of broader galactagogue evidence concluded the overall body of research was too methodologically weak to support firm conclusions for any single agent including fenugreek. Two RCTs specifically — one in mothers of preterm infants and one examining fenugreek-containing lactation cookies — produced mixed or null results, and a U.S. survey found fewer than half of mothers who used fenugreek felt it helped them. On safety, fenugreek is generally considered well tolerated at typical doses, though reviews note potential concerns including gastrointestinal side effects, possible interaction with anticoagulant medications, allergic reactions, and a characteristic maple syrup odor in urine, sweat, and possibly breast milk; researchers consistently emphasize that data on infant safety remain limited and that herbal galactagogues should not substitute for professional assessment of underlying breastfeeding challenges.
