Research suggests that Ganoderma lucidum (reishi mushroom) may offer a range of liver-protective effects, with multiple reviews reporting that its primary bioactive compounds — polysaccharides and triterpenoids — appear to support liver health across conditions including fatty liver disease, alcohol-related liver damage, hepatitis B, liver fibrosis, and chemically induced toxic injury, acting through mechanisms such as antioxidant activity, inflammation reduction, and immune modulation. A 2025 randomized controlled trial found that reishi spore oil extract was associated with statistically significant improvements in liver function markers in people with dyslipidemia, and a separate animal study suggested protective effects against acute liver injury may involve changes to the gut microbiome and lipid metabolism pathways. However, the evidence base is heavily weighted toward laboratory and animal studies, with limited high-quality human clinical trials, and findings from non-human models may not translate directly to people. Adding a note of caution, a 2012 review found that while reishi is generally well tolerated and has not been consistently linked to elevated liver enzymes, rare cases of acute liver injury have been reported, though in most instances other potential causes were not thoroughly ruled out, making it difficult to draw firm conclusions about safety in all populations.
