Research suggests that nicotinamide riboside (NR) and related NAD+-boosting compounds have generated considerable scientific interest as potential anti-aging agents, though the available evidence is mixed and largely preliminary. The studies reviewed include one small randomized controlled trial in older men, along with several animal and mechanistic studies, and their findings point in notably different directions — while the human trial found that NR supplementation raised NAD+ metabolites in muscle tissue and reduced some inflammatory markers, it also called into question whether age-related NAD+ decline in human muscle and brain is as significant as animal research had suggested. Some animal studies propose that NR may support kidney health indirectly through gut microbiome changes, while others raise safety concerns, finding that NAD+ precursor supplementation in aged mice actually worsened kidney inflammation and injury rather than protecting against it. A mechanistic study further complicates the picture by showing that NR can have paradoxical, potentially harmful effects in neurons with compromised NAD+ regulation, underscoring that the effects of NAD+ supplementation may depend heavily on age, tissue type, and the underlying health of the cells involved, and that much more research is needed before broad conclusions can be drawn.
