Research suggests that kava has a reasonably consistent body of support for its relaxation and anxiolytic effects, with multiple reviews identifying it as one of the more evidence-backed complementary options for managing everyday stress and mild to generalized anxiety, and one 2004 systematic review of 108 treatments found kava had among the strongest support of any complementary approach for generalized anxiety specifically. Studies indicate that kava's calming properties likely stem from several neurochemical mechanisms, including activity at GABA receptors, reversible inhibition of MAO-B, and modulation of dopamine and serotonin levels in brain reward pathways, as suggested by in vitro and animal research. A large 2026 survey of U.S. adults found that 88% of past-year kava users rated it effective for relaxation and stress relief, and a 2020 pilot trial observed reduced cortisol levels following one week of kava use, though both findings carry methodological limitations inherent to self-report surveys and small pilot studies. The overall evidence base is notably heterogeneous in study type — leaning heavily on reviews, mechanistic studies, and observational data rather than large randomized controlled trials — and while the safety profile appears generally acceptable for healthy adults in moderate use, rat studies suggest dose-dependent liver toxicity that is meaningfully worsened by concurrent alcohol consumption, a caution that several reviewers have echoed.
