Research suggests that iron plays a complex, dual role in cognitive function — both deficiency and excess appear capable of impairing cognition, though through different mechanisms and at different life stages. Studies indicate that iron deficiency, even without clinical anemia, is associated with poorer cognitive performance, impaired brain development in children, and reduced quality of life in reproductive-aged women, while reviews of dietary research link adequate iron intake to a reduced risk of age-related cognitive decline. On the other side, neuroimaging studies in populations including Alzheimer's disease patients, cerebral small vessel disease patients, and thalassemia patients consistently find that iron accumulation in deep brain structures such as the putamen, caudate nucleus, and globus pallidus is associated with cognitive decline, and animal research suggests that iron-driven cell death in hippocampal neurons may contribute to diabetes-related cognitive impairment. The available evidence spans reviews, cross-sectional and longitudinal neuroimaging studies, animal models, and at least one Cochrane meta-analysis of randomized controlled trials, though that meta-analysis notably found the evidence for cognitive benefits of iron supplementation in menstruating women to be inconclusive due to conflicting results — underscoring that while the relationship between iron and cognition is well-established in direction, the precise mechanisms, thresholds, and populations most likely to benefit from intervention remain active areas of research.
