Research suggests that Cistanche and its key active compounds — particularly phenylethanoid glycosides and polysaccharides — demonstrate a range of biological activities relevant to aging, including antioxidant effects, reduction of cellular senescence markers, immune modulation, neuroprotection, and organ-protective properties in preclinical models. Studies indicate that specific compounds such as echinacoside may act through identifiable molecular pathways, including ghrelin receptor signaling, TRPV3 inhibition, and suppression of stress-related inflammatory cascades like STAT1, P53, and cGAS-STING, while polysaccharide fractions appear to reduce oxidative stress in aging skin cells via the NRF2/HO-1 pathway. The current body of evidence consists predominantly of in vitro cell studies, animal models using chemically induced aging, and narrative or bibliometric reviews — with China accounting for the large majority of published research — meaning that robust human clinical trials are largely absent and the translation of these findings to real-world anti-aging applications remains unestablished.
