The most directly relevant human study is a small randomized controlled trial finding that a multi-nutrient supplement including choline was associated with modest reductions in liver fat and stiffness in adults with metabolic syndrome, while a 2025 observational study using population survey data found no clear link between typical dietary choline intake and liver function markers in the general population, possibly because everyday intake levels are too varied to produce the effects seen in controlled deficiency studies. A mouse-model study found that choline supplementation during liver fibrosis resolution lowered markers of tissue damage and improved cholesterol-related gene expression, though it did not significantly alter fibrosis-related genes directly, suggesting choline's hepatic benefits may operate primarily through lipid and cholesterol metabolism rather than anti-fibrotic pathways. Overall, the body of evidence linking choline specifically to liver health remains preliminary and largely composed of animal studies, mechanistic research, and one small pilot trial, with several of the linked studies examining liver disease through unrelated pathways such as immune signaling, gut microbiota, and TGF-beta regulation that do not directly involve choline, meaning larger and more rigorous human trials would be needed before firm conclusions can be drawn.
