Research suggests that calcium plays a complex, multifaceted role in blood pressure regulation, with evidence drawn from animal studies, computational modeling, mechanistic reviews, and clinical pharmacology rather than a robust body of large-scale human randomized controlled trials. A 1994 review of over 80 experimental animal studies found that higher dietary calcium may lower blood pressure through several simultaneous pathways — including reducing calcium entry into vascular smooth muscle cells, modulating calcium-regulating hormones, and supporting sodium excretion — while a 1989 review noted a paradox in which elevated calcium levels can raise blood pressure directly, yet low calcium intake may also contribute to hypertension. More recent computational modeling studies from 2025 add further complexity, finding that calcium deficiency raises blood pressure and that calcium regulation is deeply intertwined with magnesium, vitamin D, parathyroid hormone, estrogen, and the renin-angiotensin system, making it difficult to isolate calcium's effect alone. Overall, the evidence is mixed and largely indirect or mechanistic in nature, and while calcium-channel blocking drugs — which work precisely by interfering with calcium's role in vascular muscle contraction — are well-established cardiovascular treatments, the question of whether dietary calcium intake meaningfully and consistently influences blood pressure in humans remains an area where research has not yet produced definitive conclusions.
