Research suggests that Boswellia serrata shows meaningful promise as a complementary support for inflammatory bowel disease, with laboratory studies demonstrating that its active compounds — particularly AKBA — can protect intestinal barrier integrity, reduce inflammatory signaling, reduce immune cell adhesion in inflamed tissue, and blunt markers of intestinal fibrosis in cell and animal models. Clinical evidence is more cautious: a meta-analysis of herbal IBD trials found roughly doubled odds of remission compared to placebo, a 2015 botanical review identified Boswellia among plant-based therapies with emerging clinical support, and a Cochrane meta-analysis examining collagenous colitis found only low-quality evidence for Boswellia's benefit, rating budesonide as the more consistently supported option. Multiple systematic reviews and narrative reviews spanning 2014 through 2025 characterize the overall evidence base for Boswellia in IBD as promising but limited by small sample sizes, inconsistent product formulations, variable bioavailability, and a relative absence of large, rigorously designed trials. Studies indicate that while the mechanistic rationale is reasonably well-developed, the clinical picture remains incomplete, and researchers across these publications consistently call for more robust human trials before Boswellia can be confidently positioned within standard IBD care.
