Black Cohosh for Menopause Support

Research suggests that black cohosh has shown some promise for relieving common menopausal symptoms such as hot flashes, night sweats, insomnia, and anxiety, though the overall body of evidence remains mixed and methodologically limited. Studies include a Cochrane meta-analysis of 16 randomized controlled trials that found no significant benefit over placebo, several narrative and systematic reviews that describe the evidence as tentatively supportive but inconsistent, and a 2025 randomized controlled trial reporting meaningful symptom improvements with a combination formula that included black cohosh alongside other botanicals. On the question of safety, two independent reviews applying standardized causality frameworks each examined dozens of reported liver-related adverse events and concluded that a credible causal link to black cohosh could not be established in the overwhelming majority of cases, while multiple reviews also found no convincing evidence that the herb acts through estrogenic mechanisms or poses a significant risk to estrogen-sensitive tissues such as breast tissue. Overall, the research does not yet support firm conclusions about black cohosh's effectiveness, as most trials have been criticized for methodological weaknesses, inconsistent preparations and dosing, and small sample sizes, leaving researchers and clinical guidelines divided on whether it can be broadly recommended for menopausal support.
