The available evidence comes predominantly from animal studies and cell culture experiments, with only one small randomized controlled trial in humans — a crossover study of just ten healthy men — which found only limited immune effects following ginseng supplementation. Studies indicate that the type of ginseng extract used matters considerably, as water-based preparations rich in polysaccharides tend to stimulate immune activity while alcohol-based extracts containing ginsenosides may dampen it, suggesting that different preparations could have meaningfully different — and in some cases opposing — effects on the immune system. Taken together, while preclinical findings offer some support for immune-related effects of American ginseng, the limited human trial data, variability across extract types, and reliance on animal and cell-based research mean that conclusions about immune benefits in humans cannot yet be drawn with confidence.
