Research suggests that 5-HTP may offer meaningful benefits for migraine prevention and headache relief, with the bulk of the evidence coming from randomized controlled trials comparing it to established drug treatments rather than placebo alone. Several RCTs found 5-HTP performed comparably to reference drugs such as methysergide and propranolol in reducing migraine frequency and severity, and a 1996 study in younger patients found that treatment was associated with improved beta-endorphin levels alongside reduced headache frequency and intensity, pointing to a possible mechanism involving the brain's natural pain-dampening system. However, the evidence is not uniformly positive — a crossover RCT in children found no meaningful difference between 5-HTP and placebo, a trial in chronic tension-type headache patients found no significant reduction in headache days during the treatment period, and a Cochrane review of pediatric migraine prophylaxis characterized the overall evidence base in that population as poor quality and too limited to support firm conclusions. Studies indicate that 5-HTP's effects on serotonin metabolism are likely central to whatever benefit is observed, but variability across populations, headache types, and study designs means the evidence remains promising yet incomplete, and no single trial was large enough to draw definitive conclusions.
