Two randomized controlled trials, including one double-blind placebo-controlled study from 1990, reported significant improvements in pain, sleep, anxiety, and fatigue, while an open-label trial similarly found broad symptom improvement, though its lack of blinding and placebo control limits interpretation. Supporting these clinical findings, several reviews note that fibromyalgia patients tend to show reduced serotonin pathway activity and lower levels of serotonin precursors, providing a plausible biological rationale for supplementation with 5-HTP. However, the overall evidence base remains small, with limited high-quality trials, modest sample sizes, and one review noting that the neuroendocrine picture in fibromyalgia is complex and may involve overactivity in some signaling pathways rather than simple deficiency, suggesting that further well-controlled research is needed before firm conclusions can be drawn.
